The Stony Brook Internal Medicine-Pediatrics practice is committed to providing high-quality primary care services for the entire family, including infants, children and adults. We offer preventative care services and help our patients manage many chronic medical conditions covering the entire lifespan, including asthma, diabetes, COPD, heart disease, high blood pressure and obesity.
Our care is delivered by Medical Home teams, led by your primary care physician. We help to coordinate care to make sure that you and your family’s care does not “fall through the cracks.” In addition, as med-peds providers, we care for adolescents and young adults transitioning to adult care with chronic medical conditions.
Services
We provide preventative and chronic medical care services for entire families, including infants, children and adults. We will also coordinate care with your subspecialty providers to ensure all of our patients are receiving patient-centered care.
Transition to Adult Care
Stony Brook Children’s is committed to helping our patients smoothly transition from a pediatric model of care in which parents make most decisions, to an adult model in which the patient takes responsibility for decision-making.
Our transition services focus on preteens, adolescents and young adults, especially those with childhood-onset medical conditions and disabilities. This includes patients who have chronic conditions such as complex asthma, congenital heart problems, cystic fibrosis, sickle cell anemia, spina bifida or Type 1 diabetes. It also includes patients with intellectual or developmental disabilities, such as autism or ADHD. Transition services for these patients are intended to facilitate self-determination, build independence and self-advocacy, and prepare them to navigate the adult healthcare system.
Our Transition Services include consultations and primary care for patients ages 11 through early adulthood. We work closely with patients and families regarding the appropriate age for transferring to an adult provider and we aid in the transfer process, including helping to identify an adult provider, sharing medical records, and communicating with the adult provider about the unique needs of our patients.
Our specialists have expertise in providing age-appropriate support to preteens, adolescents and young adults, including those with chronic diseases and disabilities, who are transitioning from pediatric primary care to adult-focused primary care. Patients also receive referrals to information and resources within the community that may be helpful in supporting the patient’s personal, academic and vocational goals.
Our Team
The Stony Brook Internal Medicine-Pediatrics practice has three attending staff physicians and many resident physicians in training to serve the needs of your entire family.
Our physicians trained in med-peds complete a four-year training program that combines the strengths of internal medicine and pediatrics, allowing them to care for the elderly, adults and children. All of our providers are dually board-certified in internal medicine and pediatrics, and serve as educators in our School of Medicine, teaching the next generation of physicians as medical students, residents and fellows.

Patient Resources
Got Transition/Center for Health Care Transition Improvement is a cooperative agreement between the Maternal and Child Health Bureau and The National Alliance to Advance Adolescent Health. The organization’s website offers a wealth of information on health care transition including skills checklists, what to ask your health care provider, and how the law views health care decision-making once the patient reaches age 18.
Research and Education
Each of our physicians serves as a member of the faculty of the Stony Brook University School of Medicine. Under staff supervision, medical residents who wish to specialize in internal medicine-pediatrics may polish their clinical, psychosocial and community leadership skills in our Med-Peds Clinic.
